Certification Codes Table

Fields on a Certification Code

Enter a Code (required) of ten characters or less with no spaces or special characters.
Enter a Description (required) of thirty characters or less for the code.
An Active code has the Active checkbox selected, making the code available and keeping the code a valid choice.

Certification Information

Certification Agency. The name of the certifying organization for this certification or license is defined as the certification agency.
Renewal Required and Renewal Frequency. If renewal is required to maintain a valid certification, select the renewalcheckbox and select the frequency. The renewal frequency selections are provided by the Frequency codes validation table.
Testing Required? Test Frequency. If testing is required to maintain a valid certification, select the Testing Required checkbox and select the frequency. The renewal frequency selections are provided by the Frequency codes validation table.
US Accredited, If this certification or license is accredited by the United States, select the US Accreditedcheckbox.
Credit Hours. If hours of credit are required for this certification, enter the number of credit hours.
EE Cost. For information purposes, enter the usual employee-paid cost or fee for this certification or license.
ER Cost. For information purposes, enter the usual employer-paid cost or fee for this certification or license.
