OSHA Forms
The system OSHA Form 300 report, PER00300,
produces OSHA's Forms 300, 300A, and 301, for the reporting period designated,
and can be printed on letter-sized paper.
Generate PER00300 for the calendar year or other reporting
period needed. The generated reports include all recorded information
for the reporting period designated when the report is processed.
- OSHA's Form 300,
Log of Work-Related Injuries and Illnesses, displays first and can display
on multiple pages.
- OSHA's Form 301,
Injury and Illness Incident Report, includes how the injury or illness
occurred.
- OSHA's Form 300A,
Summary of Work-Related Injuries and Illnesses, provides the annual number
of cases per injury and illness classification. Form 300A isn't considered
complete until an authorized company executive signs it to certify the
information.
