Tier 4 Code

Fields on the Tier 4 Codes Table

• Code.  Enter a Code, of ten characters or less with no spaces or special characters.
• Description.  Enter a brief Description.

• Active?  An active code has the Active checkbox selected, making the code available and keeping the code in effect as a valid choice.
• Tier 3.  Optionally, select a Tier 3 to designate that this tier code is found only under one particular Tier 3. Otherwise, leave this selection blank.
• Supervisor (required).  Select a Supervisor code.
