Fields on the Beneficiaries Listing for an Employee Benefit Detail
Enter or modify information about an employee's benefit recipients. Some of the information on the Employee Beneficiary screen applies to defining a contact as a dependent covered under a plan instead of a beneficiary to a plan.
- Employee Number. The employee whose dependent or beneficiary is being defined is in the Employee Number field.
- Benefit. Verify or select the Benefit Code.
Beneficiary and Dependent Information
- Contact. A selected contact from the employee's defined Employee Contact Detail records designates the person who is a dependent or beneficiary and who is the subject of this record. The selection may populate some of the fields on the Employee Beneficiaries Detail.

Beneficiary Fields
- Beneficiary. An option to indicate that a contact is a beneficiary. If the employee is also a dependent (eligible for benefit coverage under the employee's benefits) select the Dependent option instead.
- Beneficiary %. For beneficiaries, enter the % for this beneficiary in the Beneficiary % field. More than one beneficiary can be set up for a particular benefit plan for a particular employee, but the total Beneficiary % should not exceed 100%.
Dependent Fields
- Dependent. An option to indicate that a contact is a dependent. The Dependent indicator will automatically populate from the Employee Contact Detail screen. Some beneficiaries are not dependents for system purposes.
- Medicare Eligible? Only used for setting up a dependent's coverage, if a benefit plan qualifies for Medicare payment (Medicare eligibility payment for a dependent), select the Medicare Eligible checkbox.
- Deny Coverage? To deny plan coverage to a dependent due to ineligibility, select Deny Coverage. This field is irrelevant to beneficiary setup.
- Coverage type information refers to dependent coverage and is irrelevant to beneficiary setup. Select whether the coverage has been designated as the dependent's Primary or Alternate coverage.
- Primary. For a dependent, if the coverage is the primary or only benefit coverage of this type that the dependent has, select the Primary option.
OR
- Alternate. For a dependent, if the dependent is covered under other insurance coverage of this type and if this is the "Alternate coverage," then select "Alternate" in the Coverage Type box to activate the Alternate Coverage Vendor information fields on the right side of the detail.
- Alternate Coverage Vendor information. Alternate coverage requires the alternate coverage vendor's general information. For dependents with Alternate benefit coverage, complete the alternate plan's information for vendor name or plan name, address, phone number, and contact representative.
